Full Care CRR assists individuals with mental illness improve skills for independent living and community involvement.
CareLink’s Crum Lynne Full Care CRR serves people who need specialized supports to achieve recovery, wellness and self-determination. The program provides assistance with basic skills, as well as opportunities for reintegration into the community.
Program Services and Supports
The program serves eight men and four women from Delaware County living with serious mental illnesses and histories of long-term or repeated hospitalizations.
Services are usually limited to two years; some residents may stay longer until they are ready for a less structured level of care. The program is licensed as a full care CRR by the Pennsylvania Department of Public Welfare.
Residential services
The program provides round-the-clock support and supervision for residents. Residents receive assistance with household tasks and learn community living skills and problem-solving skills.
The staff provides medication management, assistance preparing nutritious meals, and assistance with self-care and health care coordination. A primary counselor works with each individual to develop a personalized service plan that identifies the interests, needs, and goals of each resident. Specific goals are mapped out reflecting areas of behavior or skill development where needs have been identified.
Individual and group training is offered in areas such as personal hygiene, health care and medication management, housekeeping, nutrition and cooking, shopping, and budgeting.
Attention to recreation and leisure
A diverse selection of scheduled social and community activities is planned. Cultural differences are reflected in opportunities to attend multi-cultural events and holiday celebrations.
Residents are coached in the use of public transportation, leisure time structuring, community awareness, and strategies for obtaining educational and vocational opportunities.
Family involvement
Residents’ family members are invited to participate in their care. Family members are invited to resident treatment meetings, program parties and open house events. The staff is dedicated to promoting active coordination of care among all involved in each person’s treatment. Staff members maintain regular contact with each member of residents’ support networks.
Expected outcomes
Through participation in services at Crum Lynne CRR, a resident may expect to:
- Have assistance and encouragement to meet daily living needs
- Reduce number and frequency of hospital stays and increase stability in the community
- Learn about managing medication and coping strategies to minimize symptoms
- Develop necessary domestic skills for successful community living
- Be able to travel by public transportation
- Regularly participate in social and recreational activities
- Receive assistance to move to more independent community living when they are ready
